Doctor asked me to consider taking break from prep

I did my test in my home country 12 months ago and 6 month ago I did my test in Uk, and I didn’t have the exact report , except the consultant only tell me “ur kidney is good to go” thats all 

And today I got my results again 

Creative 1.2 mg/dL (+0.1) from a year ago 

eGFR 71 mL/min/1.73sqm (-13) from a year ago 

normal range for eGFR is 90, and the doctor say this trend is very worrying, 

say I could continue taking prep, but have to take a closer monitor, once every 3 months 

 

I personally would be going off prep, and use event based dosing , 

 

what u guys think

I'd take a break from Prep for 3 months and if you don't want to get HIV, don't bareback. Once your kidney levels go back to where they are supposed to be, which they say will happen if you stop Prep for a bit, then get back on or do the on demand prep.  I'd think dialysis would be worse than HIV but just my opinion. It’s true that dialysis would be much worse, I mean even with treatment, u are expected to live about 5-10 years, 

and u have much more trouble about daily life. 

Ive been taking prep for 3 years, 

only last year my kidney start flagging up in my test, 

Oh my given up taking loads for the time being is quite painful ? 

but thanks for your advice, I will act responsibly

Your kidneys need a rest.. go to event dosing. I have been doing that for the past two years (only have one kidney - cancer), but have a happy kidney specialist with readings in the normal range for someone with just one.

If I am reading you correctly, your baseline eGFR was 84 and you dropped 13 and now 71. Correct? Can I ask your age? 

According to a UCSF study in 2016 it was shown that those with a baseline <90 need to be monitored closely every 3 months because those individuals were most likely to show a decline in EGFR. Also those older than 50 years of age. That was my issue. My eGFR fluctuates between 70-80 and my dr advised me that Prep wasn’t for me. I still bareback and accept the risks. I’d rather get HIV than go on dialysis

i can live a relatively active and healthy life with HIV can’t do that when your kidneys are shot

Also, Descovy should be coming out soon as another option. It has shown to be less toxic on the kidneys

im in my very early 30s , 

started prep 3 years ago (on and off), only last year february it dropped below 90.. was 84 .. then doctor still give me green light for another  6months (unfortunately i dont have the exact report when i get tested at uk, due to business trip) , 

and last month i test, was 71, a drop of 13 for a guy in his early 30s, 

Ok i admit that there are many other factor that might affect my kidney as well, including poppers, 

I will cut down on poppers and prep for the time being and im forcing myself to drink more liquids before bedtime

Yes, your doctor is giving you good input when he/she tells you your trend is "very worrying."  You're way to young to have numbers like that.  Not because age is a huge factor in cause of kidney disease, it's not. i have patients in their 90's who have normal kidney function. What's worrisome is the trend.  If you dropped 13 points in a year or two, if that trend continues, you will be in kidney failure in 10-15 years. And that's assuming nothing changes to make matters worse.  You are smart to consider co-factors like poppers, apparently, they can effect kidney function too.

Dialysis happens after kidney failure and kidney failure is generally considered having a GFR <15. Standard dialysis treatment is three times a week and you spend four hours connected to a machine. It's not a perfect fix either, toxins build up in between, you have a very restricted diet. WS is out because you are not producing urine usually.  There is acute and chronic kidney disease, so one shouldn't assume that their kidneys will automatically recover if the cause is stopped... it's a wait and see thing a lot of the time.

Right now, you still have adequate kidney function, but you're smart to be paying attention and doing something about it.  Kenneth is right about Descovy, your body may tolerate that better if and when it's approved as PreP.

I think there is a lot of good advice in this thread.  I stopped PrEP  a few months ago on advice of the doctor at the clinic where I get PrEP, although it was unrelated to kidney results.  (Some blood work that was a little out of whack but which doesn't match any Truvada side effects  I can find in any of the literature.)  I didn't follow up with my regular doc until last week and am still waiting on results.  In the meantime I have to admit I have not stopped barebacking, which I know is irresponsible, but I'm shocked by how much the risk turns me on.  

If I get the all clear I will probably resume PrEP after a new round of STD tests of course, and may ask my regular doc to start writing the scripts since he now knows about the prior Truvada usage.

Hey Chargedup ,  as I aged, my risk appetite actually increases, 

guess that makes sense as the 'risk-reward'' ratio might make it more rewarding for me to take poz loads unprotected as i transit from 20s to my 30s, 

I've resumed taking UD loads from a fuck buddy, whilst off prep, it felt mentally different, and i'm a little uncomfortable to openly admit that i felt more excited (might be the little risk factor? im sounding a little more leaning towards being a bug chaser.. am i fucked up or abnormal?)

i've took UD loads while not on prep before, but coming off prep and having fun with my UD friend feels really different

First of all those numbers, although they aren't great, aren't bad.  You're a long way from Dialysis.  Before making a decision, I'd like a few more numbers and a bit more history.    What's your normal fluid intake?  do you have any other medical problems?  Hypertension, Diabetes, etc.  What other medications are you on.  Things like that.  I'd continue taking PreP but watch closely.  Increase my water intake and go from there.

Thanks, according to my latest test few days ago, kidney egfr shot up 7 points to 78 (after 2 months prep holiday) 

I'm on 8 pills a day (4 vivomixx , 4 dalfon) medication for 1 month, halfway thru the medication, I did my kidney retest, 

 

I found out that 4 of the pills im eating, dalfon ( https://www.lybrate.com/medicine/daflon-500mg-tablet ) is toxic for the kidney, 

and... vivomixx on the other hand is beneficial for the kidney (and is used to treat kidney patients with eGFR of 31-59)

I know this is serious business but my mind wondered - the thoughts been running in my mind for years - having a Doctor or Trainer - someone old while I was in becoming sexual active with males - taking me - experienced of my body ! I remember a tailor making adjustments of my designed suit - touching my crotch - he was old - wanted him to take of me and suck me off !    :)
