  1. You "sometimes hear that people have turned while on PrEP" because the tiny number of cases (2 or 3) have been blown way the fuck out of proportion by anti-PrEP fanatics. The stuff works, really well, if you take it every day. Better than condoms. (You don't see headlines every time someone gets HIV while using a condom -- and that happens every fucking day).

    Take the pill religiously and go to your doctor every three months as required, and rest assured that you're more likely to win the PowerBall than to get HIV.

    However, if you start having more raw sex than before, the tradeoff will be a higher risk of STDs. Get tested often (quarterly is best). And be honest with your wife about that risk. You are protected against HIV, and the others are curable (or, in the case of herpes, also blown way out of proportion by scaremongers).

    Three more tips: (1) Have a good long gargle with antiseptic mouthwash (like Listerine), deep in your throat, after sucking cock. You can get STDs down there and this helps kill them. (2) Try to piss after fucking raw. Helps clean bacteria out of your dick. (No guarantees, it's folk wisdom, but it seems to work for me). (3) DON'T douche after getting fucked. Contrary to what some people think, it doesn't prevent infections -- it can irritate your rectal tissue and make infections MORE likely. If you want to douche BEFORE getting fucked, do so early, and give your hole a couple of hours to recover from irritation.

    I get why some guys aren't happy with how PrEP is reducing the pool of virus out there. I decided to quit PrEP myself for my personal reasons and I'm happier.

    But not everyone wants the bug. People have the right to make their decisions based on scientific fact, not rumor. I have lots of non-chaser friends who are on PrEP and it's been a godsend for them to be able to enjoy bb like the rest of us.

    They're more likely to get struck by lightning than to get pozzed. But every time another overblown rumor goes around about PrEP failing, they freak out and it ruins sex for them again.

    Do you have any actual data to back that up, or is that just anti-PrEP wishful thinking? Truvada-resistant strains are extremely rare. None of the researchers has raised any alarm about them becoming more common.

  6. No further word from cuntboi... his last login was just a few months after he posted about his decision to let it all go. I have to imagine he finally did completely self-destruct and his life was snuffed out, either actively or passively. I hope he enjoyed himself along the way but I can't help but be sad about all the wasted years that he might have enjoyed on the future. You can be a total cumslut and keep the ride going for years -- in a spirit of joyful abandon rather than self-destruction. Seems a shame to burn out so quickly.

  7. The guy sought out PrEP because he knew he was at risk and didn't feel he could change his behavior. He was being responsible under the circumstances.

    The "shame, shame, shouldn't have done all that sex and all those drugs" attitude from some posters here -- members of a group that is devoted to highly stigmatized sexual behavior -- is really hard to fathom.

    It may be the case that this guy pushed the boundaries of what PrEP can protect against, and that most other people are not at risk of the same. But I would be careful about characterizing anyone else's sexual behavior in terms like "treating his body like a fucking outhouse." Most of us have what the straight world would consider pretty out-there sex lives. Judge not, lest ye be judged.

    I don't think that's what this is about. You can bet that of the 199,997 people on PrEP who did NOT seroconvert, some of them had slight (or not-so-slight) bleeding -- and they were still fine.

    You can drive yourself nuts reading these extreme outlier cases and spinning all kinds of scary fantasies. You might as well obsessively read about every freak car accident and worry yourself sick every time you get in a car. Could you, theoretically, get HIV while taking PrEP correctly? Yes -- but the odds are so extremely low that it simply doesn't make sense to worry about it.

    Recreational drugs can only transmit if used intravenously -- not from snorting, smoking, etc. The patient reported injecting ketamine twice, with clean needles. The researchers seem to believe him (he appears to have been scrupulously honest otherwise), and the assumption is that the transmission was indeed sexual. An intravenous infection wouldn't have resulted in the localized HIV infection in the gut that is the hypothesis. There are still many mysteries about this case, but I'm sure the researchers are well aware of the possibility of IV infection, and have ruled it out with good reason.

    And condoms are "supposed to" be used with oral sex -- it's in the CDC safer sex guidelines. These messages are always crafted to be as risk-averse as possible (often for legal liability reasons). It's just like the warnings on the package that you could die from taking Tylenol. The manufacturer of Truvada had to agree to include the condom warning in order to get FDA approval (largely because public health authorities want to keep scaring people into using condoms, to prevent other STDs).

    The study results are clear: PrEP prevents HIV infection in well over 99% of cases, whether or not you use a condom. The subjects recruited for the trials were all chosen because they were at very high risk -- most were already not using condoms. They didn't suddenly start using them once they got on PrEP -- but they stopped seroconverting. 

    You can choose to use condoms if you want, to reduce the risk of other STDs. But to say that everyone needs to wear them because 3 people got HIV out of approximately 200,000 PrEP users is absurdly overcautious.

    It's pretty fascinating -- this thing about a localized infection in his gut, never heard of such a thing. This case could potentially reveal a good bit about how HIV works. There are mysteries and outliers in every area of medicine... for example, spontaneous remission of Stage 4 cancer.

    We'll never understand everything, but we know enough about PrEP at this point to be confident that it is at least as effective as most vaccines or other preventive interventions. It's still a resounding success, even though the naysayers are going to grab every negative story to try to discredit it (something they never do with the tens of thousands of infections due to condom failure).

    The frequency is only shortening because the number of PrEP users is growing so rapidly. There are now probably upward of 200,000 people on PrEP. Three seroconversions = 0.0015%. Since the estimated effectiveness was always 99% max, that's way better performance than anyone expected. This isn't "winter," it's the small margin of error that's to be expected of a highly successful intervention that is -- like everything in life -- not 100%.

    I choose not to take PrEP, for reasons others here have shared, but I fully support those who do, and I'm tired of the constant gloom and doom about its effectiveness.

  17. In the PrEP research trials, no one whose blood levels proved they had taken four or more doses a week contracted HIV. That is not to say that you should unilaterally drop your dosage below the recommended daily use -- just that there is evidence that protection is pretty good with a minimum of four doses. 

     

    Estimated effectiveness breaks out like this:

    2 times per week would result in a 76% risk reduction;
    4 times per week would result in a 96% risk reduction; 
    7 times per week would result in a 99% risk reduction.

    Do not assume that symptoms you are experiencing are due to Truvada just because they're happening now. Correlation does not equal causation -- things can be coincidences. Nor is there any guarantee that taking just 4 doses a week would reduce side effects.

    It sounds like the safer course is to keep taking the meds as prescribed, continue to monitor your kidney situation, and listen to what your doctors say to you.

     

    As for GI side effects like loose stools: more than likely these will continue to diminish with time.

    Why don't you stop for a month or two?

    Prep take more than a week to flush put of your system. It takes about a month.

    Just be very aware of your risk factors if you stop taking PrEP. It's hard to revert back to using condoms if you quit them when starting PrEP. I know a guy (yeah, I know, anecdotes...) who stopped PrEP due to side effects and got HIV.

  19. But assuming he's poz and has a high viral load, then even without PrEP, your chances of infection are considerably less than 1.4% which is the rate for having someone with a high VL cum in your ass. 

    I think actually 1.4% is the *average* risk bottoming for an HIV+ top without protection, not for high VL specifically.   20. My experience -- your mileage may vary:

    In 2 years on PrEP I've had gonorrhea twice and syphilis once. They weren't fun, but easily treated.

     

    I feel better playing mostly with other guys on PrEP or poz guys under regular medical care -- since they're more likely to be tested every 3-6 months. But I'll admit sometimes it's hard to stay out of the sling at CumUnion and let a rando pump one into me.

     

    I'd be more concerned if sexually transmitted Hepatitis C starts to become more widespread. So far that's pretty rare but I'm keeping an eye on the news coverage.

  22. I keep reading guys here saying "I wish I could get PrEP but it's not available where I live." But people all over the world are getting PrEP using generics -- which are safe, inexpensive and good quality.
