The Transcrotal is not really a true "piercing" but rather a serious surgical body modification. This is why they are so rare.

The scrotum is a skin bag filled with some dangly bits. A straight piercing through the 'neck' of the scrotum will not epitheliate (form a skin tunnel) like other surface piercings, or piercings that pass through solid flesh.

They are constructed by making an incision on both sides of the scrotum, and stitching the boundary of the front incision  to the boundary of the rear incision, surgically forming a tunnel from the existing skin.

The sutures usually heal quickly, and can be stretched up to impressive gauges relatively quickly.

The difficulty lies in the structures that are within the scrotum that must be avoided and kept in proper orientation to the formed tunnel. Also, an infection inside your scrotum will quickly become very serious.

Anybody considering this body modification should do some serious research and try to find the most experienced (read: have done several successful transcrotals) and professional piercer possible. Consulting with a kink-friendly urologist beforehand would be ideal.

I've seen a few guys on twitter with this done and it is really hot....maybe search them out and ask their opinion/advice having gone through the procedure themselves.
