    I have been living in Berlin for a while, and its well know as the gay sex capital, but I don't get lucky as I thought I would get.

The reason you’re at risk for it is that you fuck bare, and you fuck people of unknown status. It’s also why you’re at risk for STDs like syphilis.

    This doctor sounds as though her belief standard is that prevention trumps all risk, and she would rather see you curtail your sexual choice - or not fuck at all - to ensure that you remain disease-free on her watch. She fails to understand that your body belongs to you, not her, and you alone decide what risks you are willing to take with it. She does not sound very sex-positive, nor understanding of gay lifestyles, and you might be better served by a gay doctor who understands you better. Many members here attest to having such physicians.

    If you elect to keep this doctor, I suggest you do two things:

    1) Take your damn meds. Every. Single. Day. No excuses. Do whatever it takes to make sure you get that pill in you every day without fail.

    2) Tell the doctor, politely, something to the effect of: Thank you for your earnest advice. I appreciate your concern for my well-being. I have listened very carefully, and understand the risks to me as you have explained them, and there will be no need to repeat it. I will conduct my life in a way that seems best to me, and continue to count on your care, whatever may happen.

    Yes, I take the meds, every day, since I was diagnosed in 2016. I believe in 7 years I probably missed like 7 doses, like 1 dose a year probably. Sometimes I take at 7:30pm...sometimes at 10:00pm....but Im gonna try to take it the same time, everyday ( maybe at lunch time )

    When I said "downplaying" HIV what I meant is that ---sometimes I think that --- I should be thinking like this   "  I have to stop have unprotected sex otherwise I will get reinfected with another type of  HIV Virus, and Im gonna be screwed..........if I don't  change my lifestyle, Im screwed" 

    Also my doctor told me pretty much that,  regardless what I do, eventually my meds gonna stop working and Im gonna have to change to new meds...( and I dont know why, I dont believe this. I think my meds can work like, forever, if I take it everyday, etc.....also I dont believe I can be reinfected(?)....lot of people say thats almost impossible coz the meds already come with PREP(?) so how could you get reinfected if you the Meds you are taking comes with "prep''?

    I recently had a BLIP on my viral load and I was also diagnosed with Syphilis at the same time ( and for the first time )

    My doctor is a woman, she seems to be a liberal person, at least that's what I thought...

    What scared me most at my last appointment was that she told me several times that this BLIP is the result of me not taking the medication correctly.

    She also said that it is entirely possible for me to be reinfected with a virus that is resistant to my medicines and that this will harm my treatment (and from what I have studied, it is practically impossible to have a reinfection, especially if I take the medicines every day).

    The whole time the doctor insisted on how my long-term treatment is going to fail and that my virus gonna be resistant to the medications. and that if I don't stop having unprotected sex, I will have a very serious problem

    She made me very scared about even syphilis, she told me that I could get other types of syphilis whose antibiotics don't work well

    Anyway, I'm not saying that having HIV is something simple....although I sometimes "forget" that I have this disease, but going to this medical appointment traumatized me because it seems like the doctor wanted to shock me all the time, leave me with fear

    I live in Berlin and gays here live their sexuality at full, and I don't see people here worried about having HIV or even having other diseases

    Could it be that I'm being very naive and am downplaying HIV? Unfortunately, I don't want to go back to this doctor. I felt that I was being judged

    and believe or not, I checked my viral load AGAIN ( 10 days later ) and its Undetectable and CD4 575 ( it was 500 ) before.

    I also got treatment for syphilis ( 2 penicilin injections in my butt )

    now Im gonna start with Doxy Pep as well in the future

    Hi @tomzefisThere are some red flags there already, which every single one by and in itself is unlikely to cause serious concern, but you have a number of co-factors:

    1.  You are saying you went back to your home country. If any of your female ancestors on your mother side are of Mediterranean descent please do a check for glucose-6-phosphate dehydrogenase deficiency. (I would check it anyway.) It does cause a lifelong illness called thalassemia. In your case, as you didn't have any major symptoms until now - if you test positive for G6-PhD most likely is only a minor deficiency. However, people with G6-PhD have a higher intolerance to poppers to the point of being highly toxic. It also means that you are more susceptible to infections.  

    2. Assuming that you do not have the above mentioned deficiency, the next one is Cytomegalus IgM - acute infection. This will most likely explain the drop of CD4 count. 

    3. Ibuprofen. Known to cause skin reaction if used for longer periods of time. The more common symptoms are red circular spots on the inside of your palm. They are itchy and sometimes form blisters. Stopping Ibuprofen for a while will most likely resolve it, but do tell your doctor about your intake. Strength of ibuprofen is also a factor. 

    4. Brand of poppers. Whilst it is true what @ErosWired said about poppers in the US, there are some slight variations in Europe, where amyl and pentyl variety are widely available. Heavy use, will be a co-factor in lowering your immune system barriers. So, try to limit your use of poppers for a while.

    5. Lots of travelling.... Stress due to travel would be another co-factor in your immune system being attacked. Not getting enough rest and sleep, a constant change of diet, will do a number on a healthy person and it is certainly likely to have an effect in your case.  Frequent stays in hotels, airbnbs, flights, trains, are all source of infections, ranging from minor, non-consequential to more serious allergic reactions. 

    6. If you are in Berlin, go to Checkpoint BLN. The clinic is geared towards gay men, and they will run a whole lot of other tests that 'regular' doctors in many places will not think to run. ([think before following links] [think before following links] https://checkpoint-bln.de/) It may cost you a bit, but I'd think it would be much simpler to run the tests more regularly.

    7. Finally, Covid... The 'darling' of today's news! ... sigh... Yes, you may have had a covid infection, which again, explains the drop of CD4. 

    As for the higher than usual levels of HIV, I wouldn't worry in the least, but.... Do ask your partner, gently as it may be, to do an HIV test as well. If he is already positive, run a HVL test on him too. Assuming that he is the top and cums inside you every time, it would be worth checking his stats. Since you only bottom for him, chances for you passing the virus on to him are very, very, very low.

    Other than that, take a deep breath (without the poppers LOL) and relax. You worrying about a blip can do a number on your health.

    Try to put things into perspective.... When someone tests positive for HIV, most tests would also 'detect' a viral load in the range of tens of thousands to over a million copies. So when you compare your VL of 62 with someone whose VL is 875,925. I think you can rest assured that you are still in the UD range. Of course, the lower the number the better. But variations of that scale are not a concern. Any VL below 100 should be safe.

    back in the early 2000’s when I was getting tested quarterly and tge did both the viral load and CD4 each time … … my CZd4 would slowly climb during the year and then in my January test (right after the holidays with my family) it drop - frequently by almost 50%… 

    so it could be a bad test

    it could be just a weird drop based on stress (or as EuRawBull mentioned) an unrecognized case of Covid)

    tge main thing is it sounds like you have a great dr … so what ever the case it sounds like you have the medical team to find out what happened and what tge fix is (and remember if unfit is a breakthrough… there are now lots of options to deal with HIV now

    thank you...but what scares me is that I have 62 now, after years UD....and at the same time, my CD4 is 500.....and it as always >800....so its strange that now I have a "blip" and my CD4 got very low compared to previous 800/1000.

    so Im wondering if before this blip something else happened ( like drug resistance or treatment failure )

    they changed my medication in March ( to a medication that is less likely to injury my kidneys )....in May cd4 was 1000....and viral load undetectable

    hey guys, Im on meds since 2016....always undetectable....CD4 always higher than 800.

    Today I took a cd4 test and its 500....never been this low...

    im really worried.... Im always in saunas, sex clubs...

    is it possible that I got re-infected and the meds stopped working???? I take the meds every night since 2016....

    I took the Viral Load test and its gonna be ready only in 7 days...So Im freaking now right now..

    PIG party was the best sex party I’ve ever been to. I nearly didn’t bother, as it’s rather expensive (€39) and I had not bought an online ticket in advance, thinking I wouldn’t have the energy after Horsefair.     That in itself was one of the best things about PIG- it is very well run. I only had to wait outside for about 15 minutes, and once inside there were no queues anywhere, whether checking your bag or buying a drink. Given there were apparently about 2,500 guys attending, this is quite a feat and a good example of German efficiency! For drinks, you just buy tokens in the reception area, and then exchange these for what you want at the various bars and drink stations around the club (and there are lots of them). There were helpful staff everywhere, which is why I guess it all worked so smoothly, and one of the reasons you pay €39.

    The venue itself is amazing. It’s an old mint (as in money manufacturing plant) and it’s huge. It’s spread out over 2 floors. On the first (ground) floor is the reception, coat check, bars, dance space, VIP area and a large outside area which functions as a sort of beer garden, which was great given it was a hot night. And in the basement are the play areas, which is really what everyone is interested in. It’s very hard to describe: it’s a true maze, with corridors, rooms, dead ends, more bars, more dance spaces, and random staircases leading nowhere- it’s very easy to get lost and I did several times. And hoards of men everywhere, representing every fetish you can think of, from naked to full leather and everything in between.

    Sex is happening all over, but seems to be divided somewhat into rooms/zones. There’s a fisting zone, a piss zone, a cumdump zone (where I naturally gravitated to), an oral zone, sling rooms, shower rooms, odd rooms with random bits of stuff in- whatever you can think of, it was there somewhere. Of course, it’s not just sex- there are loads of guys standing round drinking, socializing, dancing- but it’s all kind of mixed up together, which is great.

    I actually found the experience quite trippy at some points. The sheer size of it and the random layout, the slightly creepy ‘abandoned factory’ vibe coupled with some weird lighting effects and strange artworks- there’s a real Alice In Wonderland feeling to it.  For me, it’s the ultimate Berlin experience, and encapsulates everything that Berlin is known for.

    Honestly @tomzefis, if you actually live in Berlin, you should definitely go next time it’s on. The Lab is great too, but PIG is on a whole different level.

    My viral load has been undetectable since 2016 and I never missed 1 pill of my medication during all those years.

    I want to start bareback.

    Is there any chance for me to get Reinfected in a way my medication is gonna stop working? what are the most updated information about that ( I only found out an article from 2002 )

    I will be in Berlin...and Im 41, and not the perfect body....I have a belly that Im not proud of, even thought Im not really fat

    I heard is really difficult to get f**** in Berlin if you are not young and slim, so in my case what parties / places in Berlin should I go to get more chances to get F****?  I really dont wanna spend all that money and going back home without nothing 😞
