It would be interesting to know how long the Bexsero vaccine is good for gono partial protection. You get two doses (the first dose at month 0, the second dose at month 6). But do you need a booster? Like every 5 years? -

Basically I use on-demand PrEP about twice or three times a year. I don't use that many tablets (maybe 30 tablets a year tops). Just received the bloodtest results, and kidney filtration function appears to be excellent. Nothing else to emphasize in the rest of the blood test. So in my case, on-demand PrEP doesn't seem to affect kidneys.

Yeah, but my question is: how likely is it that the blood test is gonna detect something weird that could be related to me taking PrEP?

Hello, I take on-demand PrEP (Truvada) through a specific GP (specialised in STDs) and the last pill I took was on Saturday, November 9. Now, I have to do a blood test for a completely unrelated reason (I have to test for glucose and kidneys) through my regular GP. The blood test is scheduled tomorrow, on November 30. How long is Truvada detectable in the blood after last intake? My regular GP doesn't know anything about the fact I take Truvada, and I wouldn't like to disclose to him that information. Is Truvada gonna be detectable in my blood 3 weeks after the last pill?

According to the WHO, VMMC (voluntary medical male circumcision) reduces the chance of HIV infection by 50-60% for heterosexual/insertive sex. There are biological reasons to believe these findings also apply to total tops. Hepatitis C is pretty difficult to transmit sexually, especially if HIV negative.

I pretty much agree. FWIW, I'm on Truvada, not Descovy (because Descovy is not covered by insurance where I live). But it's not really a problem as I almost don't experience sides with my modified Truvada on-demand regimen. I have heard that Descovy was better tolerated than Truvada in most people.

Yes. Starting four days before with a reduced dose, taking the pill with food and crushing the pill with my teeth before swallowing it seemed to help. I think it's a combination of all these three factors because food or not, the initial double dose seemed to be overwhelming for my stomach.

The reason my physician allowed me to go on this regimen is because I am a circumcised total top. In his view, in my situation, even a low level of PrEP in my system would be sufficient to prevent a potential infection, provided I took care of other STDs and cuts/abrasions. The initial double dose is particularly efficient for bottoms though, he said. So I guess I will just stick with this modified on-demand regimen + DoxyPEP if needed

Okay so here is my current on-demand PrEP regimen as discussed and modified with my physician. My main problem seemed to be that the initial doses (especially the first double dose) were too much for my stomach. So instead of the 2-1-1 standard on-demand regimen, I was allowed to use an alternative 0.5-0.5-0.5-0.5-1-1-1 regimen. That is: - Four daily 0.5 doses on the four days preceding the sexually active day (to get the drug progressively into my system without hurting my stomach too much) - One full dose on the sexually active day - One full dose on the next day - One final full dose on the following day Doing this (and always crushing the pill with my teeth before I swallowed it) seemed to reduce my gastrointestinal issues to almost nil while maintaining a good level of protection. I post this for information only, always do things with the permission of your physician.

Hello, I recently tried PrEP for the first time, on an on-demand basis (2-1-1 dosage). And it was just awful. It felt like I had the stomach of a 80-year-old. I couldn't drink, even one beer. And even if I took the pills with a meal, I experienced pretty nasty stomach discomfort. What's the point of being immune to HIV if you can't even enjoy to party and play? So I stopped the medication, and quickly recovered (about 2 days after stopping medication). Now I am talking about alternative options with my physician (maybe going for a 1-1-1-1-1 dosage to decrease side effects). Anyone else experienced stomach issues linked to on-demand PrEP?

FYI, my doctor confirmed to me that the 2-dose vaccination scheme against mpox was effective for a lifetime. Just got my second jab.

Hello everyone, We all know the hepatitis A/B vaccines provide lifetime protection after 2 or 3 doses. But what about the HPV vaccine (gardasil, 3 doses) and the mpox vaccine (jynneos, 2 doses)? Do they provide lifetime protection, or will we have to get booster doses after 5, 10 or 15 years?

You are entitled to your own opinion, but I humbly believe this is slightly exaggerated. In fact, some staunch Republican supporters are gay, like Peter Thiel or Scott Presler. Being LGBT+ doesn't require us to be woke advocates or to support a specific political agenda, especially in the free West. This is not Russia or Saudi Arabia. I voted for Clinton in 2016, and did not vote in 2020 btw (liked Trump foreign policy but at the same time didn't want to vote for him). But the problem with the current Dems is that their aim is not to DO GOOD THINGS, but to APPEAR AS THE GOOD ONES. And that I cannot stand, especially after the Biden-sponsored genocide in the Middle East: at this precise moment, for the first time of my life, a vote for Trump wasn't out of the table. So yeah, having mean tweets in exchange of world peace and prosperity seems like not that bad of a deal honestly. Just my two cents, not gonna dwell on this topic, this is my final message.

Because most gay/bi guys aren't total tops. Circumcision only protects total tops, and said protection is incomplete (about 60%). -
That's a perfectly understandable statement. But believe me, we exist (lol). This is not a political website so I am not going to elaborate on "why", but it's interesting to see that even here, we find some diversity in that regard.

Hi again, So here is my final strategy to avoid HIV/most STDs. Maybe it could inspire some of us here: - Lube - Vaccines against hepatitis A/B + HPV + mpox + meningitis B (the Bexsero meningitis B vaccine offers a 50% protection against gono) + normal vaccines from calendar - Circumcision - Avoiding high-risk behavior (e.g.: no receptive anal whatsoever as I am a total top). Because hepatitis C is easily transmitted this way and there is no vaccine against it (curable but a pain in the ass) - PrEP on demand (on vacation as I only bareback on vacation) + DoxyPEP + full testing (STDs, kidneys, liver) twice a year I will keep you updated on this thread in case I still catch something despite this comprehensive strategy. -

If you bottom bareback, you're at risk for hep C. Period. Unfortunately there is no vaccine yet but the current treatments make it curable in about 8 weeks time. -

Spot on. According to clinical data, DoxyPEP reduces odds of syphilis and chlamydia by about 90%, but only by 50% or so for gonorrhea. Gonorrhea is no fun, but a new oral antibiotic against drug-resistant gono has been found: [think before following links] https://www.nytimes.com/2023/11/10/health/gonorrhea-treatment-zoliflodacin.html No need to get a sting up your ass like with current treatments. Moreover, therapeutic HPV/herpes vaccines are currently prepared and should be released in 2025 or so. Finally, as I am a circumcised total top, my risk for HIV and genital herpes are already lowered by 40-60% (but with PrEP, HIV is nothing to worry about anyway).
