Prep Providers List

i live in Chicago, south side. I was on PrEP for a while, had to go up to the Howard Brown centre, a great place with great people, but would like to find someone closer. Is there a list of PrEP providers in cities? I asked at the local medical centre and they had no clue what i was talking about.
