Whether the top slips it off mid-fuck and stealths the bottom, either the bottom or the top sabotages it so it will fail during the fuck or whatever other devious, creative ways someone comes up with to make sure the seed gets planted! I'd imagine we'd see lots of big girthy cocks with the shreds of destroyed condoms at the base...! (Or maybe a super size cock could simply have an undersize condom slip off inside the bottom to be found later....) Going from holes in the condom, Vaseline/oil lube to break it, stealthing. Sometimes as a bottom I'll pretend I'm a little too tight to take the dick, and the top would often go soft, then I'd remove the condom to suck it back hard then suddenly jump on it ot tease my hole until the top pushes in.
