Wild!  How does it work on the inside for the top?  is there a piece that gets inserted in his urethra?  Or is there nothing inside, and the only part is on the outside?

The ones which are sold do not have any inward inserts. 

Normal use roughes up the top real good too though. I often get a bloody penis (especially around my head's edge) from it when I top with it, which nicely enhances any DNA exchange.

 I have made a whole collection of them by now and also modified some to be even more disruptive.

Most of them are geared toward outward destructiveness with the bolts being a real good 'additional tear up' 

I had one guy using them reverse on himself with the bolt into his uretrea . It did cause a bloody mess in my ass... I loved it.
