I've got the Mr S Leather VAC-U-LOCK Leather Harness. It wasn't cheap but tops that have worn it with my larger toys say it's comfortable to wear and very adjustable. The only issue is that it's pretty much only for vac-u-lock compatible toys. But most of the larger ones come ready for that now anyway.

I never used to cage myself when I was getting fucked, but I've found that doing so makes it clear that I don't want my cock played with. The only 'problem' I've had is that my hole opens up really easily now, so even if just being fucked I can get sloppy pretty quickly. But honestly I'd much prefer that than being too tight or uncomfortable. I've never had any other issues. Never been incontinent or any similar issues.
