You have to take it daily for up to at least  

2 weeks or 3 weeks to build up on your system enough to protect you.

Yes, I've done a few years ago.

I took one pill a day and started three days bevor my sex vacation.

Today I would start at least four days bevor the first sex date.

I'd start about a week in advance to be safe. If you have less time, there has been a study that showed taking a double dose 24 hrs in advance of sex than taking it daily was still pretty effective.

Starting a week before should be plenty of time to get the blood level of the Truvada up... I know a guy who does this regularly for his trips to Ft Lauderdale.  While he's in Canada he doesn't have sex... comes down everyother month for a couple weeks...

Same here. I'm not having much sex partly because of HIV. I can't see myself on PrEP in a long-term basis because I wouldn't be able to justify taking the drugs if I'm not having enough sex.

I'm planning a vacation to a Spanish-speaking country in a couple of months so I have enough time to decide and start with enough time. I'm staying in an apartment where the owner provides a portable sling. I setup a profile where I advertise myself with a sling and interested in anon sex, pump & dump type action. In spite of being older than 60, I've received tons of winks and messages from guys who want to stop by the apartment for anonymous action. From the responses and previous experiences there, it appears that many of these Latino guys are into mature guys. I love Latino guys! This made me think about PrEP.

I'm not a big fan of PrEP because it does not protect from other STD and I know that being on PrEP I would want to have more anon sex and other STD would show up. I also have a problem with PrEP because I don't see it as a long-term sustainable solution because of money and long-term side effects. But I woulkd like to go on PrEP for maybe 2-3 months. A few weeks to get used to the drug and evaluate the side effects and then for the rest of my 6-week holiday.

That's a perfect situation for taking PrEP intermittently.  That's what is great... You can go on and off PrEP as needed with no diminished effectiveness... Have a BLAST

Canada is a wonderful country! Same-sex marriage, medical marijuana and soon its legalization. And now getting PrEP seems fairly easy. The sexual health clinic that did the testing called me back and told me that once the HIV test result comes back negative, they will make an appointment with the Dr that will prescribe the Truvada. Because I'm in Canada all they needed for the Dr's visit was my Health Card information, zero co-payment or deductible. I'm not sure what will happen with the price of the drug itself. My insurance would probably cover it once it's prescribed by a Dr.

It HIV blood test comes back poz, then obviously no PrEP. Although I'm not a chaser and I have no interest in pozing I would be able to console myself by having lots of slutty sex while in vacation.

On Friday got a call from the Sexual Health Clinic that my results were back and everything was OK. Today Monday I got a call from the specialist to offer me an appointment for this Thursday to discuss PrEP. All I need to bring is a list of medications and my Health Card.

I agree that money is a serious drawback. Long-term side effects I worry less; the specific drug substances that are in Truvada are known to have a favorable side effect profile. In fact, I believe that is part of the reason why it is the drug recommended for use as PrEP.  My experience (nearly continuous use at the recommended dose for about 3 years) has been that the only measurable side effect is some stool loosening when my diet doesn't prevent it. That said, everyone's body chemistry is different, so YMMV.

You can look at the data that Gilead gave to the FDA yourself. It's summarized in the prescribing information; if you go to the NIH web site here https://aidsinfo.nih.gov/drugs/406/truvada/8/professional#nlm34084-4

Scroll the navigation pane on the left down to the bottom and select "adverse effects", then scroll the text pane down to Section 6.2 - it gives the actual data from the (5+ year) clinical trials (about 3000 people in each trial, half got Truvada, half got placebo, and it shows the rates of side effects in each group). The rates are virtually identical in the two groups... therefore we can conclude that the chances the side effects are from the drug are very small.

It's important to note that this is the data from people who were NOT infected with HIV and were NOT taking other antiviral meds. Data from trials on people in those groups are elsewhere in the document.

Event-based dosing – IPERGAY and other studies

One of the most widely publicised ways of using event-based dosing, relates to a French and Canadian study called IPERGAY.

IPERGAY dosing

The dosing in IPERGAY involved:

Taking a double dose of PrEP (2 tablets) before sex. This would ideally be the day before, but could be anywhere from 24 to 2 hours before.
Taking another single dose (1 tablet) after sex – i.e. the same day or 24 hours later from the last sex.
Taking another single dose (1 tablet) the following day. - or 48 hours later from the last sex.
This means taking four tablets if you had sex once in a week. This also means taking four tablets if you had sex lots of times if this was all on one day.

If this involved having sex on several days, then daily dosing was recommended, with a final dose the day after all this activity ended.


*** Tenofovir takes up to 24 hours to reach good levels in rectal tissue.
*** taking PrEP the day before sex is likely to be better than relying on taking this just two hours before. 

If planning to rely on event-based dosing for infrequent use – i.e. less than weekly use – then there is a good reason to be cautious and start PrEP the day before.

There is a reasonably effective post exposure medical treatment that one takes for 28 days after a probable exposure to HIV. You must start this treatment within 72 hours of exposure and the sooner the better. Ask your doctor or emergency room about PEP It is not as effective as PrEP but works well.

Are there any long lterm serious side effects from prep? I just started 2 weeks ago.
