Some guys have longer, slimmer Cocks, and can relatively easily rut in a tight Hole.  Some guys have shorter, thicker Cocks, and can't easily get into a Hole that hasn't already taken a few loads.  

Thus, it's up to the Top to use what he's got to best advantage.  For guys that own the latter type, some tongue-work is in order to open things up a bit, assuming the bottom hasn't been taking much yet.
