I have no issues taking poz loads but I still have hesitation when someone tells me they have syphilis or some other STI.  I am on Prep and take doxy, and I do eventually accept their load but I would like to lose that hesitation.    Does anyone else have this bit of doubt? If so how do you get past it?

Up to you, but don't have to be alright with the chance. Doxy pep is good at stopping bacteria STDs, I plan to start it soon, but it's not like the almost certainty prep gives you for HIV protection. And it costs you if you get something, it takes time to get tested and treated, something like gonorrhea can be 2 weeks of feeling crap.

HIV is one thing as there isn't a known cure and only treatment you have to take meds the rest of your life. STI's, especially gono, need to not be fetishized, and hurt those that receive it from others. It ruins the fun of care free sex, and sucks having to go through the treatment process. I ended up losing a month and a half play time last year because I caught gonorrhea twice within a months time.

Unlike when this issue first appeared (meaning hiv), today there are preventative steps we can take (or not) to, at the very least, mitigate, and for the most part, reliably prevent hiv (as well as the more common irritating std's) from being transmitted.  It's not impossible, of course, but the chances that PrEP and Pep offer us of remaining rather healthy from these stds' is - at least to me - well worth any bother.

Obviously, nothing is "guaranteed", but the chances are greatly reduced that illness will come knocking on your (back) door from taking loads off as many cocks as you can.  However, when weighing the reduced chances of disease, against the pleasures to be derived, the fulfillment of your most lewd needs/requirements, and live to tell the (thousands of) tales, PrEP & Pep present a fantastic opportunity to maintain one's health and be the man you were born to become.  You've taken all the available preventative steps, and worrying about the lack of 100% mitigation won't make it a fact.  We take a chance of getting run down just by crossing the street;  risk is always present.  You've done the most there is to do at present, so get that hungry hole to your favorite fuckjoint and pig out 💦

When I hit the fuckjoints, I can honestly say that catching something is about the farthest thing from my mind, since I've taken the steps available to mitigate illness.  I see that you have as well, so let the issue go - you've done everything you can at present - so hit the fuckjoints, shuck your jeans off and steep yourself in the wanton whoring.

Let's not forget about TasP.  Before PrEP the one known preventative, identified by multiple partner studies; is Treatment as Prevention.  It is one of the reasons that even off PrEP, the risk of getting infected is still quite low.  In recent years TasP was rebranded to U=U.

For years the other STI's just showed up every two or three years.  And while inconvenient; fairly easily dealt with.

I think that since the advent of PreP, and PEP, especially if you take your  bareback sex seriously, you needn’t worry….wish I had that in the early 2000s. I get the anxiety around not wanting to be Poz. But it’s better than dying? Like so many of my friends have.

But is it carefree though? Ever since I became Poz, I have been cognisant of the fact that I was careless in my sex choices. We still have the choice of not partaking in certain sex acts…I went to a sex cinema and there was this guy who had his pants down to his ankles showing a rosebud fuckhole lubed with Vaseline petroleum jelly….which would have rendered a condom useless…man, I would have loved to bend him over and fuck him but there’s always the thought:will this be the fucker who will give me chlamydia?  The hole reeked of fuck juice and sperm, but there is totally no way to prevent stds , except for DoxyPreP.

Doxy pep isn't even that effective. Really the only things we can do is test, and I'd recommend testing monthly if you are having sex with a lot of random men often enough, or if you have any symptoms. I try to be an ethical slut, but I understand my risks.

There’s also choosing not to fuck with a stranger in a situation one can’t control. …which was my point about things not really being carefree.
