What is the purpose of the former sling room/orgy room space now? It has a raised platform taking up most of the space now. Sometimes guys still play a little in there but I haven't seen more than two at a time.

So this doesn't sound too bad and the projection is getting out to where it is dangerous to project further (I am well aware of the projections already reported in the press). However, if the model I induced holds, there are likely to be > 200k cases by Christmas. There is a small chance that the outbreak will die out on it's own but I wouldn't bet on it (although the CDC seems to be doing exactly that in their "wait and see" heavy approach).

There are. But they are a minority of str8 people (how much of a minority is not clear though). I was reading a blog recently where the author stated that it was only recently that he realized that the majority of Gay men don't have nearly as much sex as he thought. He's correct. The majority of people on this board who are sexually active tend to be much more sexually active than the average Gay man, although this has to be considered for age and region/city as well. At this stage in the outbreak it appears that transmission is broadly occurring through sex (although at least one Gay man went on a party weekend , traveled with his two partners, neither of whom came down with monkeypox but he has classic symptoms - he did have sex at one or more sex parties but only with his partners he was traveling with).

No one has died yet in this outbreak (and the strain is the less virulent one of the two) but people are experiencing severe illness with serious pain if they come down with pustules. Cumulative cases in the US are now just over 1000. The virus is spreading exponentially. Unless the outbreak is contained (which looks unlikely) , the US will have 10,000 cases around August 25, 2022 and about 20,000 around September 12, 2022 (I know because I induced a model of the cumulative case curve - this is not the first time I have done this and my projections are correct within a couple of % - cumulative cases tend to be easy to accurately project in the early stages of many uncontrolled epidemics). As for COVID, > 1M people have died from this virus in the US and > 6M have died worldwide.

They are giving a monkeypox specific vaccine. I have read that there is a problem/cause for concern giving the smallpox vaccine to immunocompromised people.

The German page says when several of the German states are rolling out the vaccine (beginning today even though some has been distributed already) and who to contact. Germany has already distributed some vaccine but the real roll-out begins today. I was going to suggest that you might also be able to get it in the Netherlands but the English page says "by invitation" so presumably they are primarily vaccinating through contact tracing (if so, that will be dropped). That's true. However the article also says that among the set of people who experienced PrEP failure, some 13% had followed the protocol correctly. So a naïve estimate would support a failure rate of 39/10000 (3% * 13%). This is also much higher than expected but inline with protective %'s cited publicly - 99% effective - this article suggests 99.61% - if this is approx. correct and the time period is a year then PrEP provides 79% protection over a 60 year period. But the article can be read as limiting protecting from testing quarter to testing quarter which then reduces the protection to 79% protection over 15 years.

Occasionally in Germany after sex I might be asked if I were Jewish (I'm not). I usually responded that almost all American males are circumcised at birth. Sometimes I did say I had Jewish ancestry to see where it might go, which was nowhere, at least not with the people I met in Germany (I was living there so this wasn't a short term thing). So at the end of the day I never directly saw racism (except in the US, but that's another story). People are tripped or triggered over different things. On my first Gay trip to Belgium a guy I was banging afterwards asked me if I was a priest - because I had short hair (I was in the military at the time). I was really taken aback by that one.

I saw a tweet saying that New York state health officials say that there have been monkeypox cases in people born before 1972, people who had been vaccinated against smallpox (routine smallpox vaccination ended in the US in 1972). Currently there are 11 cases in NY, 10 in NYC. Another tweet said that there is some limited data published by the CDC showing ages of monkeypox patients ranging up to 76 - the median age is apparently in the mid-30s). Basically if a person was born in 1969 or before, they were given the smallpox vaccine without question. Some people born between 1970 and 1972, but before the abandonment of universal smallpox inoculation, did not get the vaccination.

So one way this is instructive is the revelation that there are two major different kinds (clades) of monkeypox. The one in this case study is the West African one which has a mortality rate < 3% (and is mostly children and immune compromised individuals [which may or may not mean older people]):

I was surprised to find out that monkeypox is presenting in a way different than classical cases. The lesions look quite different. As it turns out there have been numerous medical papers on monkeypox outbreaks in the West since at least 2006. Here is a recent case reported in the New England Journal of Medicine - a Gay man presents with monkeypox following sex in "southeastern Canada". No source thus far has released any report of the number of people infected. One source from 6/13/22 said that 1 person had been infected ; other sources said "a handful". The source that said 1 person had been infected from IML specifically said that the # of cases in Chicago at that point was 7, which was an increase of 5 cases from a pervious period. However another more recent source said that Chicago now has 13 cases of monkeypox. Therefore the number of people likely infected at IML <= 7 (the 1 person reported to have been infected at IML + the increase in cases afterward). This will be inaccurate esp. as the incubation period of monkeypox can be two weeks (a Dutch study shows that the mean incubation period is 8.5 days) so more cases might roll in. That is the efficacy rate published for new vaccinations. They never seem to have updated the rate because smallpox was wiped out. There have been questions about the efficacy of the vaccine over 50+ years.
