    Hey all - I think I remember seeing comments from an RN or a PA on here - hopefully you can chime in. Got my latest tests (every 3 months) and all was as expected except positive for throat gono (Pharyngeal Gonorrhea)...ok, so I LOVE sucking dick. Urine and rectal gono were both negative. Dr didn't note any need for treatment - so I thought,,,why not? From what I've found, it appears the gonoccoci bacterial cannot thrive in the throat like they do in urine and rectal locations...resulting in 50% cure rate within 1 week and 100% cure rate within 3 months with no treatment. WTF? So although protocol is to treat all three locations to stop a spread to other partners, the reality is that if I don't have sex for the next three months I don't need treatment...is that right? AND, in addition, two studies show that gargling full Listerine (full alcohol version) reduces the chance of getting a positive result by 50% even during those three months where it would still be active. Lastly, even though some strains are resistant to penicillin and doxy they still recommend a 7 day 100mg twice a day Doxy regimen as a first choice to shorten the cure duration.     Could be throat gono (Pharyngeal). Sadly, I only guess this because I went on a tear last month and now tested positive for it. I've most likely had it in the past as I remember feeling like I had a cold or sore throat after fucking around. Doc says protocol is no treatment since I was negative for rectal and urine gono (if you have two or all three they treat). Normal process is that it goes away after 1week to 3months duration. I'm kinda pissed at myself for getting this, I was being good for a year, just had to slut it up. I've read that gargling Listerine (has to be the good stuff 20% alcohol) before and after giving head drastically reduces getting it and shortens the duration to kill it off - so I've been gargling twice daily - go deep (:) with the stuff as it's the back of the throat that gets it and later gets swabbed. Anyone else out there dealing with just throat gono with any other info on shortening the cycle? (I'd like my next test 3 months from now to be neg).

    The CDC says the chance of contracting HIV through oral sex is very unlikely. The factors that could allow the transfer of HIV infected blood or semen would be open sores in your mouth, open sores on his dick. If you can, go on PrEP (I'm getting it from overseas for $28 a month), get immunized for HPV, HepA, HepB and pneumachoccol. Then get a prescription for doxycycline (or get it overseas). Doxy has two preventative roles for STIs. 200mg taken within 72 hours after a sexual event reduces chances of syph and chlam by 47%, 100mg taken daily for a week before a crazy sex weekend party and then continue taking for 4 weeks after reduces all STIs by 78%. This "1 week before through 4 weeks after" is the same protocol for preventing malaria, so there is no adverse effects or any chance of reduced effectiveness.

    Back to the doxy as PrEP and PEP: A series of studies showed a 100mg pill of doxy, taken as PrEP DAILY showed a 70% reduction in Syph and Chlam, no change to Gono instances (but Gono instances were already low), but the same approach but only with occasional use of doxy as PrEP (before sex) showed nearly the same reduction but lessened the possibility of resistance. A new study of 200 mg doxy as PEP (after sex) showed a 47% reduction in both Chlam and Syph. For Gono itself, it is transmitted on average to 124 people per 100,000 population. Below is what it does IF it was never treated. In men, gonorrhea can cause a painful condition in the tubes attached to the testicles. In rare cases, this may cause a man to be sterile, or prevent him from being able to father a child.

    The studies using Doxy were conducted with daily Doxy of 100mg and then 200mg - which definitely raises concerns about resistance, but occasional preventative use is a feasible tactic to reducing instances of STI's - as one person noted Gono is already resistant to all but one antibiotic.

    If you haven't had any type of Hep, you should get a Hep A vaccine and a Hep B vaccine. You should also get an HPV vaccine and an pneumococcal vaccine (covers mono and others). Hep C has no vaccine, is sexually transmitted and has an 18-20% full recovery rate (ie, stays acute then you're immune). If it becomes chronic there's a cure but it's expensive. And, studies are showing that a 100mg pill of doxycycline before sex reduces the chances of syph, gono and chlam by up to 70%

    I wonder myself. At first I thought I'd be embarrassed to hit the lab every three months - the techs obviously know what's up with the regimen of tests I take - but then I wondered how the pharmacist would react if I picked up PrEP - but I didn't have to worry about it since I'm buying it from overseas. For a bi guy who was never open to anyone but my wife I've been surprisingly open to discussing everything with my doctor. It doesn't help that some of the latest commercials for PrEP are for "those dealing with HIV". I keep thinking, that's not accurate, why are they promoting it that way?? Then I'll see another commercial that shows all the stereotypical folks (us included) who are "at risk" and it discusses PrEP as a way to stay safe...on target, of course. Weird that they're sending two messages like that.

    Just for everyone's FYI - I ordered Tavin EM from Dynamix (USD$69 for 90 day supply) and the process was fine. Got my written script on 9/10, ordered on 9/13 (you need the written script and a scan of your passport), was shipped out on 9/17 (from Singapore, made in India like all the others including Truvada) and arrived today, 9/21. I'll wait until Oct 7th to start, that way I have about a month overlap (get the next written script then order, ship and arrive with overlap so I don't run out...).

    Yeah, I've been getting tested every 3 months for over a year - had to deal with syph, (it's dropping fast) and Hep B (I take tenofovir and might have to for awhile, so instead of JUST that, I might as well switch to PrEP to get the tenofovir + peace of mind). My doctor is cool, she's also going to fill a script for 100mg of Doxycycline (100mg before fun or 200mg after fun if it wasn't expected) to prevent syph, gono and chlam. Both Green Cross and Dynamix get theor stuff directly from India and it's been tested, so should be OK. I've read that the generic Truvada coming here next year might still be $500 a month Also, the tenofovir I take (Ten DF) is the same as in PrEP - and is used to treat both Hep B and prevent HIV - but we've all heard of the new Tenofovir (to eliminate the kidney issues and bone density loss) that isn't out yet...hopefully it will be in a newer version of PrEP soon. I'll give Dynamix a try.

    Trying to figure out if either Dynamix or Green Cross (or both) are reliably delivering to the US - I read some reviews that weren't so complimentary. Truvada in the US is approx. $1100 monthly (I have good insurance but I make a lot of $$ so no free or massively discounted PrEP available to me) Generic from Dynamix is approx. $25 monthly - 3-4wks for delivery (only requires my written script and passport info) Generic from Green Cross is approx. $60 monthly - 2-3wks for delivery (only requires my written script and passport info)

    Here's some updated info. While PrEP only prevents HIV, you can take 100mg of Doxycycline - either daily or before an event to minimize the risk of chlam, gono and syph (200mg AFTER fun helps but better to be preventative)- I already have HSV1 and 2 and have had maybe one flare up over 20 years with it so no worry on that front for me). You can get an immunization for HepB (which I'd have known that before I contracted it), for HPV and for Pneumoccochal. Hep C is your biggest foe - there's no immunization and although 30% clear up on its own, the other 70% requires expensive three hit treatment. Just get on PrEP, get all the immunizations possible and go have fun.

    Well, for FYI - I've got chronic Hep B now because I hadn't even thought to get vaccinated during all the years I played (bipolar, not diagnosed so not on meds, therefore I didn't give a shit about anything or anyone). If the coming year of Tenofovir DF doesn't drop my HBV viral load to zero, allow the antibody to form and kill off the antigen then I'll have to take it everyday - the good news is that Tenofovir DF is half of PrEP, so I already told my Dr I'd prefer to just go on PrEP since it will provide the HepB treatment AND HIV prevention. - For anyone dealing with kidney or bone issues from Tenofovir DF there's good news on that front too. The new Tenofovir ( Tenofovir Alafenamide) does not cause kidney problems or bone density loss and is in final studies phase to replace Tenofovir DF in PrEP. My dose of TenDF is 300mg, the same as it is in PrEP so that's why I would be able to switch to PrEP, but the new TenA will be only 25mg for the same affect. The drugs are evolving and requiring lower dosages.

    My doctor recommends Prep every day, said she is not comfortable with on-demand scenarios because you can easily forget, not have access to it or run the risk of lowered effectiveness. As a side note, she gave me an HPV immunization, a pneumococcal immunization (mono, etc), a Hep B immunization and agreed with my question about preventative doxy to prevent Syph, Ghono and Chlam (100mg daily). So with the daily doxycyline , the daily Prep and the proper immunizations you can drastically lower the chances of everything but Hep C.

    Hey there - I knew what you were saying (I've been to the pleasuredrome, the one in Soho and all the Chariots!) - I was a complete slut before I ever thought of getting on Prep (and am still right at the edge of the "pull the trigger" phase). Once I'm on Doxy and Prep, the world will open up again and I'll continue to be the sluttiest slut I know - so for me the meds will just ensure I'll be around/mobile/healthy for my kids.

    To make you feel a little more at ease - I took hundreds loads in bathhouses, video stores, dark basements and alleys for almost 20 years without prep or anything else - and I'm still neg. And I don't even have the CCR5 deletion to explain it. So, if you've been on prep for a few months, HIV is almost a non-issue - Syph, Gono, Chlam, Hep B, HepC and HPV are your only concerns.

    Thanks - it's the right thing to do since I have kids. I also just got a Pneumococcal Polysaccharide immunization yesterday (prevents against meningitis, bacteremia and types of pneumonia) - why not, right? Truth be told, for 20 years I never gave a shit what might happen to me - feeling amazing during each romp but terrible after. Being diagnosed bipolar this past year explained a LOT so 200mg of Lamotrigine every day has given me the clarity to prepare for the types of fun I know I love. Now to figure out if the wife understands.
