As a bottom, I've never had this happen to me, but there's been times when  I get home and realize how close it was to happening. I clean deep, but the colon has twists and turns and it's possible for water to go in to a part of it and not come out immediately. For me it seems to be size.  Both length and girth.  I can take multiple tops in a lengthy session without an "incident".  But whenever a really hung top plunges in deep, it can get messy.
