Actually, I know people who do that kind of thing at restaurants all the time, and restaurants that honor those requests.  (One is a friend with severe digestive issues after prostate radiation treatment; he'll ask for a dish of plain pasta with just some parmesan cheese sprinkled over it at Italian restaurants, and they're usually happy to provide it and invent a price for a dish not on the menu.)
