The City does have high treatment and PrEP rates, but it also has a small population (about 800,000 people in the City proper), high HIV prevalence and a particularly active sex scene, mostly bareback (as it should be!).Locally, the infection rate is considered high. It would be interesting to compare infection rates in other cities, and to assess whether the variations stem from different levels of sexual activity and barebacking, or different health initiatives. Increases in infections among youths and people color are worrisome. As a top on PrEP, I like to help with outreach by inseminating bottoms. It's a serious responsibility. :-) -

Time of day and day of week matter a lot. In my experience, there is action just after work (5-7 PM) on weekdays, early Saturday and Sunday mornings after the bars close (2 AM- ), and on Sunday afternoons. On the contrary, the regular STI testing that doctors are supposed to order with PrEP helps PrEP users to detect and seek treatment quickly for this minor inconvenience. Anal chlamydia is often asymptomatic for bottoms, and due to shame or anonymity, the tops -- who are more likely to notice penile chlamydia -- aren't likely to notify their bottoms, either. Men without the regular medical attention that comes with being on PrEP (or being Poz and in treatment) are likely to go quite some time before addressing miscellaneous STIs. Someone once told me about a guy who never intends to break tax laws, but believes that, if the (U.S.) Internal Revenue Service doesn't audit him, he has left money on the table. So it is with sex: if you don't get the occasional STI, you aren't having enough sex!

Just a PSA, which I mention often because the standard of care varies so much by location and venue (gay-friendly, competent, and resource-rich or not):Always insist on a 4th-generation HIV antibody test. This reduces the detection window to under 4 weeks. If you are afraid after a risky exposure, insist on a PCR RNA HIV viral load test, typically a "qualitative" (present/not present) rather than "quantitative" (how many copies of HIV?) one, although quantitative tests are increasingly sensitive. This will tell you within as few as 10 days after exposure. This reduces worry, and the information could save yours and your sex partners' lives. The sooner you know, the sooner you can avoid transmitting the virus and the sooner you can begin treatment, minimizing damage to your immune system (if treatment and not transmitting are your goals). In public health facilities in San Francisco, this is the standard of care and you could leave with an anti-retroviral prescription the very same day you get a positive test result. -
I thought I'd post this publicly, for the benefit of other readers. PEP, called Traitement post-exposition or TPE in French, is widely available in France. The French standard is 4 to 48 hours after exposure, and non-occupational exposures are acceptable. During business hours, some clinics offer TPE; otherwise, any hospital emergency room does. The protocol in France is an evaluation, a 2-to-3-day prescription, another evaluation, and then a 28-day prescription. By law there is no cost to French and EU citizens, but you would have to inquire about the cost to foreign visitors, which could be substantial and might or might not be reimbursed by your regular or travel-related health insurance plan. Another strategy, albeit not ideal, would be to enjoy a few bareback orgies in Paris and immediately avail yourself of PEP. Post-Exposure Prophylaxis is a 30-day course of a full HIV drug regimen, and is effective in preventing infection if started within 72 hours of exposure. It is an international standard in emergency room medicine, although if you were relying on it you'd want to verify availability at a specific French hospital, coverage/payment terms, and the risk that your medication would be seized by customs when you returned home. Unlike PrEP (as currently approved in the US and pending approval in some other countries), PEP entails a choice of HIV drugs. To avoid resistance issues, US guidelines recommend choosing the drugs based on the regimen of the positive sex partner, if known, but that obviously wouldn't be possible with multiple, anonymous partners.

The research -- particularly the iPrEx-OLE study -- is in your favor. Take Truvada daily and you are protected. Be sure that you have been vaccinated for Hepatitis A and B. The HPV vaccine can't hurt, either, though it is most effective in people who have never been exposed (i.e., have never had sex before). On a separate note, asking for testing paperwork is not a useful protection strategy. Paperwork would be easy to falsify. Even if it is honest, you might not know the type of HIV antibody test actually performed (4th-generation tests detect antibodies within weeks of infection, whereas older tests take months) and you cannot know the person's sexual behavior during the window period or since the test. Paradoxically, a positive top with an undetectable viral load could be the safest possible partner. (Preliminary results from the first two years of the PARTNER study show no transmission by people with an undetectable viral load in the last quarterly test, to their negative partners, even without condoms or PrEP.) A negative top could well have been infected before or after his last test, and since he wouldn't know it, he wouldn't be in treatment and could have a very high viral load.

I topped bare and also swallowed loads before PrEP, and I do both even more freely, after 2 years on PrEP. I've had no noticeable side effects. In terms of miscellaneous STIs, I am required to test quarterly at this point (and the FDA and CDC guidelines for Truvada for PrEP call for this) but I've kept up my initial testing schedule, which was monthly. Not surprisingly, I've found fewer exposures when meeting individuals (e.g. through an app.) than when going to a public sex environment like a glory hole place, and also fewer exposures in suburban areas than in San Francisco proper. I talk up PrEP to any Neg or unknown-status bottom I fuck. -

Get PrEP from KP San Francisco rather than South San Francisco. It is worth the trip. You will find a world of difference.
