According to him the person who gave it to him is someone who is NOT on meds because he doesn't even know himself he's HIV+ It sounds a bit strange because from the post-mortem writeups I've seen the most likely route of the multidrug resistant virus would be someone who was failing Stribild treatment, given the very specific profile of this guy's virus resistance. So if that holds: a) either infecting guy is lying and he knows he's poz, got some meds and taking them poorly or b )maybe it was someone else, guy says "it was a busy period"

Not much of a mistery. When the bottom gets a load with tons o virus in it, the chances of the virus making into the blood through the tears of fucking is quite high. You get the load staying around your guts with tons of virus just waiting to find an entrance in you. Bottoming without ejaculation has far less risk, about the same as topping. BTW, guy was a bottom and (very likely) seroconverted from a load someone failing therapy on Stribild.

TL;DR Guy on PrEP with good adherence contracts multi-drug resistant HIV strain. Infection is caught right away and guy is now UD with his current regimen.

Been on PrEP 3 and some years. I've got fucked and bred in all sorts of settings. From one on one hookups at home, saunas, darkrooms, to cumunion parties. Most of my loads come from saunas, darkrooms and cumunion. I can easily get 5 loads per visit, and in these 3+ years I'm sure I've got 200+ loads in me, from absolutely anonymous cock. I've also got poz UD loads that I know about, but I just don't ask anymore. I have NO interest in getting HIV but I know PrEP is just ridiculously effective. I'm still negative, pretty sure I would have been pozzed by now if it wasn't for PrEP but I wouldn't have been the slut I am now without it either. Just go on PrEP and forget about HIV. It's more likely you'll die in a car accident than getting HIV.

Let's see. HSV seropositivity is so high 75%+ in gay men, that it's sort of pointless to tell people about it because they probably already have 1 or 2. Also, You can get 1 in the genital area, or 2 in the mouth, and otherwise. It just happens that 2 is often more in genitalia and 1 in the oral area. Also, if you take 1mg of Valtrex daily you will almost eliminate your viral shedding. Do that and there's no need of telling anyone. -

You may want to ask for valtrex. It's a different prodrug of acyclovir that has better bioavailability and it's only 1X day to prevent outbreaks. -
Regarding IPERGAY, it's not clear at all the benefit came from intermitent dosage. The people on the study were so slutty that 'on average' they were taking 4-5 pills per week. We've seen before that +4 pills/week is close to perfect protection, so their protection could come from accumulated levels over time and not from the just taken pill. Also to answer the OP, the usual talked about regime is: 7 days to 'load up', fuck like no tomorrow, stop, 30 days to wind down sort of like a PEP regime just in case.

So prep seems to offer close to perfect protection for gay men, less so for other groups. So prep breakthrough seroconversions can happen and resistance is a concern. No one should live their life thinking prep is bulletproof. Nothing is.

The way euro goverments are delaying the introduction of PrEP to save a few euros is sad. Europe could approve PrEP today and take the cost of brand Truvada for a couple of years until generics arrive yet they choose to delay it until they get the cheap stuff, and create all these 'trials' to prove what's already been proven ad nauseaum. Thanks to France! for seeing that this doesn't make any sense, and that approving PrEP now is the right thing to do, economically and sensically.

This new study further makes the case for PrEP. Some people were concerned that if you seroconvert while on PrEP you'll develop resistant strains while taking PrEP. This seem to have happened during partners PrEP, a straight study of PrEP users. Seems like if you simply stop PrEP after seroconverting the resistant strains will disappear and you'll get regular strains flourishing which are vulnerable to normal drugs. Being HIV+ is defined as having established a HIV viral reservoir that to this day, is an irreversible, for life status. PrEP works by stopping HIV from establishing this reservoir so your body fluids can't infect others while on PrEP. Others that fuck you getting infected from foreign high viraled loads is a theoretical possibility...

I have really good news for you. Gilead changed the copay assistance program so they will now reimburse without monthly limits up to $3600/year. So fill that RX at the beginning of the year and Gilead will pick up all your deductible and you'll be even done with medicals deductibles for the rest of the year.

Like everyone else I pay zero. I was actually paying $10 every three months with my obamacare plan. I felt it was ridiculous to ask gilead to pay for the the $10 when the refill is ~$4K but eventually decided they should just cover it all with the copay card and use money for a beer

He 'may' have implied that you've chosen not to use barrier protection for rimming which is where hep B is a problem. In that context the comment could have had a more clinical nature... I'd cut a lot of slack to a doctor who's 70+ working at a catholic hospital anyway.

The USA model is not that great either. There's heavy reliance on people going to their regular doctors for prep but most doctors are clueless about it. With the French model it's just about getting people to the right center. How common are sex clinics throughout France?

Was reading more about it and seems they'll have to go through specific clinics and hospitals. This could make it difficult for some people to go on it vs the USA model where any doctor can prescribe. If that's the case it will require a lot more outreach and it will just be more difficult in general to go on it.

There's zero concern regarding cipla trustworthiness. They are a huge generics manufacturer with a good track record. The weak link is still the online pharmacies. Testing drug levels is as good as its going to get for a while. If enough people keep getting tested for levels we can sort of reliably white list online pharmacies...
