This article has mostly gone unnoticed but it seems to provide a plausible way in which PrEP may just not work for some people. http://www.aidsmap.com/Study-sheds-light-on-how-tenofovir-is-processed-in-different-body-tissues/page/2985533/ Some people genetic makeup make it difficult to metabolize Tenofovir properly in certain areas of the body. Article talks in particular about women and vaginal tissue, and how 8% of women tested had gene makeup that suggests inneficient Tenofovir processing leading to improper levels for protection. It's murky from the article what happens in rectal tissue and if there's a segment of subjects who also struggle to process Tenofovir properly in those areas. A bright point of this is that if if true, we could actually get tested before starting PrEP to know if it would be protective for a particular individual or not.

Let's assume that all the PrEP breakthrough cases floating around 2 or 3 are real, all of them show resistance to emtricitabine. This could be because: a. Original infecting virus was resistance to this and therefore PrEP was less efective b. Infection happened and person developed resistance through the use of the only two drugs in Truvada I think a. is unlikely because guys on meds are not infectious and guys NOT on meds don't usually have resistant viruses. I think b. is a lot more likely and reinforces the need of getting tested frequently. Actually in one case on the Partners PrEP study (straight study) this was conclusively the case. Person developed resistance by taking Truvada while being infected. I'm assuming we'll hear sooner than later about the PrEP breakthrough cases with carefuly study and they'll tell us the whole story.

When I used condoms I had them breaking on me many times. Specially when lube was not properly used and guy had a big cock. Some guys have really big cocks and condoms just rolls up into your ass. Sometimes this happens after they've come and you end with a load in your ass anyway which would have been the same as fucking bare. If I condom didn't break, didn't slip, were used all throughout the session.. you may be looking at 100% protection for HIV. Real world usage brings their efficacy down to 70%-90% ...

HPV is extremely congagious and condoms are not of much help for not covered areas. This is for example why pubes are there, to avoid HPV and other viruses from being passed on. I'll never understand why some guys shave completely. I think your body can also clear gono and chlam on its own, so lots of folks must get infected and clear automatically. It's the infections that stick that are problematic. I get tested very frequently, therefore why I may have caught things early. Syphilis is not fun but easily treatable you should get tested often. And Herpes I think most guys get it eventually, both HSV-1 and HSV-2, it just doesn't seem much of a big deal for most people. For some, with recurrent outbreaks there is Valtrex that works very well. -

Got chlamydia twice in the butt and once in the mouth in this timeframe. Condoms would have probably avoided the one in the butt... Chlamydia is the most common one apparently (after HPV that we all have and noone gets tested for). Previous to PrEP I had got gonorrhea in the mouth once, so I'm sort of living proof that the 'there are other STDs out there' argument against PrEP is bullshit unless you are using condoms too for cocksucking, which I don't know anyone doing it. -
I'm a PrEP veteran I guess, been doing it for 2.5 years. While I was sort of selective of who I'd let cum in my ass before, it has now become into a 'sure! breed me if you want' attitude without much previous discussion of anything. Without any discussion actually. I'm now getting fucked raw much more often and the number of loads I'm taking has increased substantially. Not sure if I should be doing this, but it's fun. Have you guys started getting fucked more/taken more loads >1 year into being on PrEP? My baseline before PrEP was definitely bad in the 'safer sex' realm but it's really bad right now.

Cost seems to be the problem in Europe. I would like to see exactly when we could get generic Truvada in Europe. I've heard some people say 2016? USA seems 2017 is the year, but first generic versions may not be much cheaper than the brand name. Just to reinforce bearbandit's comment, guys tested today only know they were negative 3 months ago, and if infected they are likely very infectious right now without them knowing. I usually think that if you are having this conversation with your sexual partners is a good thing, but not a super effective way to keep HIV away. Real risk reduction things you could do is avoid getting loads, but still getting fucked raw, o you being the top most of the time. Obviously PrEP would be fantastic for you but it´s not available quite yet in the UK.

Statistics. The more loads you take the more likely you are to get pozzed. PrEP reduces your chances 99%, but if your chances are high to start with you can still get pozzed.

Count me in. Seems like I'm going through phases of total sluttery followed by celibate season because I get afraid. I guess we are not used to the freedom of having sex the way we want it. At least for me, I've been having sex with men for 15+ years with the fear of HIV always present. You can't expect that fear to go away overnight specially when you add all the nuances of PrEP protection. It will take a while for everyone to settle in their personal happy medium.

Actually I had thought someone in Europe or somewhere should pick this one up. Buy generic Truvada from online pharmacies, check the stuff in the lab to make sure it's the real thing, and 'certify' the pharmacies from time to time. This can't be official in any way, but still much better option than not having PrEP available at all, which seems the current status quo in Europe. If Europe/Australia can't make the finances work they should allow exit avenues for people at risk to get on PrEP somehow.

Had a situation today where a hot couple who are both 100% tops (yeah right) wanted to tag team on my ass. All was going fine until I mentioned I was neg on PrEP. Answer was "we are neg but use condoms 100%" Should I: a ) Try to convince them they are at zero risk. I'm on PrEP, tested neg 2 weeks ago, they are the tops. Very low risk for them. b ) Let it go (obviously not getting fucked with rubbers. I'd rather just suck cock) If you feel I should try a) what points should I bring them up to sway them to drop the rubbers?

For How Long Insurance Companies Are Going To Cover Prep?

Don't remember the name of this insurance company, but they already backtracked. They are going out of business and they are trying to save every single dime. Insurance companies that are concerned with the cost of Truvada have Prior Authorizations that require to make sure this is given out only to high risk groups. Insurance companies are not stupid and if they can give Truvada to the right people, virtually anyone in this forums, they'll save money long run. -

There's clearly a threshold where if you give PrEP to the right people you are going to save money. Treating poz people for a lifetime is going to be way more expensive than providing PrEP. Where is the threshold though? I did see a canadian study which did the math and for PrEP to 'save money to the system' you would have to assume it's 90% effective and carefully give it to those who are at real risk of getting HIV.. if you start giving PrEP to the worrybees who only suck cock you are going to lose money for sure. Like you, I'm pretty sure all these countries are waiting for going ahead with cheaper generic versions.. otherwise, it just hard to justify the cost. -
Some people compain about having the runs.. but if it's Truvada caused it goes away in week. If not, check with your doctor

Side-effects and cost. If you are HIV+ you are going to put up with side effects just because you have to, but for prevention the side-effect profile really needs to be very favorable. There's also the cost issue.

Guys who dont know they are infected don't have med resistant strains, are therefore they will be stopped readily by PrEP. I should look it up, but seems med-resistant strains are not virulent. Add the two, and the case of being infected by someone who's carrying Truvada-resistant HIV sounds very implausible. The biggest problem by far with PrEP is starting it while having just serocoverted. This could very well be the case recently hitting the news; guy didn't get RNA tests on baseline and could have been infected. Antibody test went negative for a while while he was a few months on PrEP and eventually he developed resistance to the drug... If you go on PrEP demand RNA tests, a couple or three weeks apart, while you are starting. You do want a solid negative baseline. This one has been talked about before. The big thing for us to notice here, is that these people were straight, and likely women. For women TDF concentrations in the vaginal area are orders of magnitude less than in rectum, therefore more prone to seroconversions. Also It can't be established how well adherent the people who serocoverted were. They were definitely taking it sometimes but there could have been gaps, we don't know.

You have to take a load from someone who is unmedicated and with high VL. Sounds a long stretch. Case looks legit, but he mentions that "I was very ill for about two weeks. I had chills, night sweats, and a horrible cough when it was done. The flu was rampant at the time so I didn't really think much of it." Given his VL was so low <1000 it's just hard to picture why would anyone would have a fuck-flu when they are essentially undetectable. Either he's a phenonemal Bullshitter or just the first known case to get infected while on PrEP.

I think anyone, even a NP can issue a prescription. And all you need is quarterly checkups for STDs and kidney. It's totally cool to educate your doctors or NPs about this. They are 'general' practitioners and they don't know everything. The one that thing you really want, is that they are supportive, so the first doctor doesn't sound a good match. Just look for anyone in the clinic and if not go somewhere else.

If people can't cough up the courage, some STD clinics provide an 'anonymous' service where they'll call or send a postcard from the clinic saying that someone they've been with tested positive for an STD and they should get checked. I frankly don't have problems telling the tricks I care about. When I broke the news in the past the reaction has been positive. It makes stronger the trustworthiness in the person.

Yes, you got chlamydia in the ass from one of those 4 raw fucking instances. Chlamydia, is the most common bacterial STD. Urine test will only detect an STD in the urethra, not in the mouth or in the ass, you need all areas swabbed. PrEP protects you from HIV but nothing else. The good news is most other STDs can easily be treated with antibiotics, so the important thing is to keep getting tested and treated. I'm sure doctor gave you antibiotics that will take care of any chlamydia you may have. It would be cool that you informed your 4 partners about this, because it's 100% sure one of them is infected and should get treated.

HPV vaccination could still be good regardless of what you hear around that 'if you already got HPV vaccine will do nothing'. Not true. HPV infections are localized and are not blood-borne. While the vaccine can't do anything about a current infection in some area it can still prevent infection in other areas. Also, for Gardasil-9 you may very well have not been infected by any of the 9 types it covers so you'll get immunity to that. Regarding Chlamydia in the butt, most common symptoms are none. Often, the body fights it off and goes away but sometimes it can cause trouble. That's why it's important to get your ass swabbed for gono and clam. -

I see I answered this thread a while back, but I'll update. Now, I've been on PrEP for 2 years and a half, (does that make me a PrEP veteran?) Never had any side effects. Took many many loads, and getting fucked raw a lot more often. I can't remember last time I used a condom. Still negative. I did get chlamydia in the butt a couple of times, but went away easily with just one pill (1 gram of Azytromicin). Also HPV is lurking around without much consequence, but HPV is unavoidable if you have any sort of sex... I'm pretty convinced PrEP worked saved me from seroconverting because a lot of the loads happened from random people in bathhouses.. seems like I should have seroconverted by now.
