This is a great reason to use it on demand, but also to realize that you MUST be careful about potential HIV infection because that would mean you would have to take medication everyday. With that said, the newer formulation of tenofovir (tenofovier alafilamide) that is used in the current HIV meds has far less liver and kidney side effects than the version used in Truvada (tenofovir disoproxil). The studies of using the newer version in Prep are currently in progress, but they need to be studied seperatly because tenofovir alafilamide has a much shorter halflife.

QFT. Prep on demand in thoery works great, you are going to hoe out, you do your two weeks before and after, and you are good, but this the poster I quted said, many people will thing a couple before, and a couple after will do it, and yes it will provide some protection, but the amount is not well known or studied. For example two weeks ago I got a call from a guy that I fucked three weeks prior, and had tested neg 5 weeks prior, what did he say? he tested poz. He wasnt on PrEP so undoubtedly he had a sky high viral load. While I am consistent about my PrEP usage, it still scared me. People in general are fallible, and I dont have that much faith in people taking a drug because of something that exists only in theory. All of the PrEP studies have said that one of the key issues with it is taking it consistently. So why would that change for on demand dosing when you are not used to it vs consistent usage? That also leads to another issue, and thats of comfort. Take the guy I just fucked who tested poz. Before I was NEVER worried about him. he was a complete top, who got tested regularly, even if he wasnt on PrEP. So If I wasnt on PREP that could lure me into a false sense of security. Maybe I had just enough pills on me for a big hoe out,but not a quick fuck, and knowing that he is honest and reliable, I fuck him without taking my PREP because its always been okay. Then without knowing it I just fucked with a very infectious person. To me the idea that you can plan out sex completly, and be 100% certain of your actions in a given situation is a bit naive. People make mistakes and thats what PrEP is there to protect against, but it cant protect you, if you dont take it.

Good article, and also a little bit of a wake up call for those on here that think HIV is some unbridled panacea of sex. People who are hiv+ or hiv- do fall in love with the opposite status, and it’s not always an “anything goes” type of thing.

Edited some stuff out, but This is a great post. To the OP, I want you to think about something, and thats a life without sex. For many reasons, men sometimes either stop having sex, or cant because of medical or physical reasons. Think about that life if you are HIV+ or HIV-. I say this because life has tons of ups, downs and changes. Right now sex is a fairly important part of your life, and there is a good chance that may not always be the case. While HIV is pretty much a non-issue in terms of life span today it still drastically complicate things, From things like life insurance, possibly having a kid, to paying for medication. You have to make your own decisions on this, but I would strongly suggest you think about your ENTIRE life, and everything that goes along with it, and think about that life WITHOUT sex, but WITH HIV. I think you may find that staying on PrEP may be a good idea. This forum is great for lots of things, but in general I have found balanced arguments lacking, and for the people that will say, "Yeah dont worry, get pozzed" Thats like saying to an addict, "dont worry about it, do all the drugs your want, you will make it out." Yeah there is a good chance you will be OKAY, but at what long term expense, or complication to the rest of your life? -
There is no easy answer to this, for some people they may have a detectable viral load in a matter of weeks for other people it could be years. The viral load is also a main indicator of how infectious the person is, and again, that will vary from person to person

Is there a reason he isn’t on treatment yet? For his sake going on treatment ASAP is a good idea, even if he is stable without it. For your sex life it’s an even better idea. Being undetectable is key in not transmitting hiv.

Talk to your doctor or medication manufacturer about assistance, there are many programs out there. As the previous poster said stopping meds is a very bad idea. The idea of "med holidays" was popular back in the late 90's and early 2000's back when there were more severe side effects, unfortunately they always always ended up with increased resistance. HIV mutates very rapidly and the reason the meds work is that the virus doesn't have the ability to mutate if it's constantly surpressed. medication resistance is very real and a very bad thing, especially wiTh long term injectable meds coming on the market. They are based on current drugs but last 2+ months with a single shot. However if you become resistant to certain drug types now, you may be SOL for those drugs which would almost certainly make life much easier. in short do not take a break -

ding ding. Heres the thing, and you probably don't want to hear it, but you just got unlucky. Most people are exposed to CMV and other variants and acquire some type of immunity by 40, and most much earlier. Think about how many people get Mono, I had it at 18. Unfortunately with these things everyone reacts differently. When I had mono, I was just a little rundown, but I know people that were in the hospital for two weeks. You are still on the right track with PrEP, and please stick to it. With CMV, you were probably going to be exposed at some point anyway, depending on the timing your bodies reaction may have been different, but who knows. If you want to take time off go for it! There is nothing wrong with that, but I also wouldn't be afraid to return to sex, and definitely not be afraid of undetectable guys when on PrEP, they are very safe.

Thats true, and misleading. There are many proven studies so far that say 4x a week dosing provides protection very close to that of 7x a week dosing. With that said, I wouldn't do it. As for the original question I just wouldn't do it. IMO thats a quick way to mutated HIV strains.

This is all well and good except for the fact that there is no proof it works. If it really works, I would say you should post your numbers and how long you have been HIV+ for.

Of course, but really it speaks more about the unwillingness of many people to discuss things like HIV status with real language. And really Ive seen guys of both status' play into it. Also let be real here, NO ONE is asking if someone showered when they ask, "are you clean?" They are asking one of two things, did you douche? and or "whats your HIV status?" Playing stupid with the answer really doesnt help anyone in the long run. Even if the question may be difficult for some people.

Many canadiens have found doctors who are prescribing Truvada as PrEP as off label treatment. Bingo. PrEP with Truvada is still incredibly effective. Its also important to note that even vaccines are almost never 100% effective. As a biomedical prevention method PrEP is still incredibly effective. This case was going to happen eventually. The fact that in 3+ years of PrEP usage we only see one documented case is a testimate to how effective PrEP is.

Actually that is somewhat false, at least based on official surveys and statistics. Here's some of the links on statistical data and modeling. It includes data broken down by race. What is happening is that GBM are having sexual relations much more so only within their own demographic. Because of this any STI's including HIV are amplified within GBM as a subgroup. When you combine that factor with the fact that GBM tend to live in areas with lower socioeconomic status, and have less access to healthcare its a perfect storm for high rates of HIV infection. With the proper care, HIV is hard to spread. People who get tested regularly, and if infected are on treatment are highly unlikely to spread the virus. However, a person who is undiagnosed even one year is VERY likely to spread the virus to sex partners. The issue has become even more skewed with the affordable care act because many black people, including GBM tend to live in the south and other states that choose NOT to take federal funds for medicaid expansion which would have covered many people with quality heathcare for free. For example, you have one GWM, and one GBM who are both versatile. Both become infected at the start of a one year period. In the next year the GWM gets tested and on treatment at 3 months, and goes on to have 30 sex partners the rest of the year. During the same time the GBM has 20 sex partners, but doesn't get tested. Because he has been highly infectious for a longer period of time, he is at a far greater chance of spreading HIV, despite having fewer partners. Additionally because he is more likely to only have sex within the smaller subgroup GBM, he is more likely to infect other GBM. Now its important to remember that this is all based on numbers, and it doesn't take into account things like feelings of passions or the idea of being "uninhibited" For statistical purposes the length of the sexual act doesn't matter. A three min no talking pump and dump is that same as a three hour all over the house passionate fuck fest, as long as sperm goes in a hole.

Well I said it, so yeah I definitely feel like there is. Racism comes in all forms and can come from people of all races and backgrounds. In this particular forum there is an incredible amount of fetishisation when it comes to black men. I dont think that automatically means someone is racist, but maybe those people should ask themselves some questions like, Would I date a black man? Would I feel comfortable bring him home to family and friends? Do I have friends I know are racist? Am I thinking about black men only as objects to fuck me? etc etc. In some cases ones answer may mean they are racist. Look at ANY website/app, and its easy to spot rasicm in all forms, white looking for same, latin looking for latin or white only, black looking for black only, etc. People have their own attractions based on looks and thats fine, but too many people lump what they call "attraction" into institutionalized racism. That comes in the form of white guys not wanting to date/fuck black guys cause they think they are all "thugs" Black men not wanting to fuck white men because they think they are all racist assholes who will never get them, or practically every fucking gay man not giving any asian guy a shot because they think they all have small dicks. None of those things have ANYTHING to do with attraction, its racist tendencies that no one wants to admit to. People don't want to admit the uncomfortable truth about what they are. When it comes to black/white relations in America, just talk to someone of the opposite race someday about racism, I assure you it will be an eyeopening conversation. There is a lot of growth and honest conversation people need to have to realize that racism isnt just not calling people slurs and being cordial to them, Its having real conversations, and a genuine desire to improve the relations of all people. I don't think that fucking another race hurts relations in anyway, but what helps more is having a constructive dialogue, and learning how everyone can improve themselves. Lastly racism isnt a cut and dry, "yes I am" "no I'm not" thing. This is why its so important that all people be aware of it and try to learn and improve themselves.
