If you increase the amount of fiber in your diet, digested food will move through you more efficiently, cleanly and thoroughly, so you can be sure you really are "empty" after a good bowel movement (and maybe a quick douche of the lower rectum). While I avoid a big meal in the hours before a fuck (or several fucks), I certainly don't starve myself. A boy's gotta keep his strength up, after all :-) About your second question... relax. You can't be always ready. There are threads here about that too. While spontaneous is fun, you sound like someone who really likes to be ready and prepared, so plan your fucks around when you're ready and prepared, or plan your preparations around your scheduled fucks.   6. In the U.S., daily dosing is the recommended protocol and that's what I do. Sex isn't always "planned" and you might not have the two pills with you to take in advance. If you (with help from your health plan or insurance) can afford it, I agree with you that daily dosing makes more sense. The studies I'm aware of show Truvada-as-PrEP very effective through daily dosing. I'm not aware that it has yet been proven to work just as well in "on-demand" use. I hope it will be (for people who can't afford daily use or for people who have infreqent sexual encounters) but would want to wait for that to be definitely shown and approved. While you're deciding, consider how much sex you anticipate having in future and how planned that sex will be. And don't rely on how much you have been having since you said you've put off sex while waiting for PrEP to be available in France. That may help guide your decision. One last thought: About Truvada being a "heavy drug." It's not. The reason it was chosen to test for use as PrEP in the first place is because it is so well tolerated by those taking it. Very few people have any side effects and of those, most are mild and go away in the first few days or weeks. (Yes, a very few people have bad reactions and can't tolerate it but they are a tiny percentage (<1%). You are HIV negative and want to stay negative. I applaud your decision to protect yourself.
  7. It's always disappointing when the build-up is WAY hotter than the actual experience. I enjoy sharing our mutual likes/dislikes, fantasies, etc. ahead of time, but too much "planning" what we're going to do often fails in reality. Now I try to keep my expectations in check and just go with the flow to avoid disappointment. Been there too many times.   8. I get tested for all STIs every 3 months both to stay healthy and protect my partners. Since I'm on PrEP, I'm more concerned about getting chlamydia, gonorrhea or syphilis than I am of contracting HIV. I don't want any STI and I sure don't want to pass one on to a fuck buddy (many of whom have become friends). It sucks to have to tell guys you've fucked with recently that you've found out you have something and they need to get tested or treated.
  9. What are you using to douche? If it's some kind of squeeze bottle or hot-water bag, invert it and give it a few moments for the air bubbles to rise and dissipate. It's it something like a shower-shot, I haven't had that problem. I agree, pumping air into your hole can cause cramping (and farts) so I avoid getting air in me when cleaning out.

  11. Lube -- tons of lube, way more than you think you need -- will help. Don't hesitate to pause for a couple seconds to reapply if it starts to dry out. And maybe put off your own orgasm until you sense the end of the session. A lot of bottoms seem to find it hard to continue getting fucked after they cum. I'd avoid drugs like meth (for a host of reasons), but a drink with your FB before you start might help.

Current recommendations are this: If you're on PrEP and want to stop (say during a period when you're not expecting to have sex), you should continue taking your PrEP pills for 28 days after your last high-risk (condomless) sexual encounter. This is the same length of dosing as what's recommended for effective post-exposure prophylaxis (PEP) which is taking HIV medications to prevent infection after an exposure event.
